All international students in F-1 and J-1 status are required to have the NAU student health insurance, which will be charged to all student’s LOUIE accounts each semester .
Exceptions will be made only for students who meet one of the following criteria:
1) J-1 students who have insurance coverage that is backed by the full faith and credit of the government of their home country and that meets the J-1 Exchange Visitor Program requirements. To be eligible for the waiver, students must also be from a school that accepts NAU exchange students into a program that does not require NAU students to purchase local health insurance. Policies from private insurance companies, even if backed or endorsed by the government of the home country, will not be accepted.
2) J-1 and F-1 students who are sponsored entirely by the government of their home country, including health insurance coverage.
3) J-1 students who are sponsored by the U.S. government or an international organization and are provided health insurance coverage.
4) J-1 and F-1 students who have insurance provided through U.S. employment or through a parent or spouse’s U.S. employment.
Requests for waivers must be submitted within the first ten days of the start of the semester to the Center for International Education. Requests submitted after the first ten days of the start of the semester will not be eligible for a waiver.
How to request a waiver
If you meet criteria #1: Please provide the following documents to the Center for International Education within 10 days of the start of the semester:
1) The completed exchange student health insurance waiver request forms, and
2) The completed and signed Verification of Health Insurance backed by the government of your home country, and
3) A photocopy of your health insurance ID card and the policy, including dollar amounts of coverage in English.
If you meet criteria #2 or #3: Please provide the following document to the Center for International Education within 10 days of the start of the semester:
1) Photocopy of the front and back of the health insurance card provided by the sponsoring government or international organization.
If you meet criteria #4: Please provide the following document to the Center for International Education within 10 days of the start of the semester:
1) A photocopy of your health insurance card showing your name and dates of coverage.
If your request is approved, the waiver will be granted. The final decision to waive the NAU health insurance requirement will be made by the Center for International Education.
Requests for waivers and refunds of NAU health insurance for previous semesters will not be granted.
Note: Before requesting a waiver, please keep in mind that Health care in the U.S. is extremely expensive and most medical facilities do not bill out-of-country insurance providers. Payment of the medical bill is the patient’s responsibility. The patient will need to seek reimbursement from the home country insurance provider. Health insurance is required for your financial protection and for expenses of the health care providers. The university is not responsible for medical expenses or filing claims.
